Notice of Yunnan Provincial Hospital of Traditional Chinese Medicine on completion certificate for advanced students

Dear teachers,

If the teacher finishes the training as scheduled, please prepare the materials and go to the Clinical Practice Teaching Management Office (601, 6th Floor, 6th Building, Yunnan Provincial Hospital of Traditional Chinese Medicine, No. 22 Yunrui East Road) to complete the training and completion procedures.Prepare materials as follows：

1. Scan the following materials into PDF files, the files are uniformly named: name + name of the unit, and the electronic scanned files are sent to the training group (recommended scanning software: Scanning Almighty King)。

1. Application form for further study: Please fill in "Personal appraisal" as required, further study department fill in "Department performance assessment and appraisal", and have a signature and signature stamp;
2. Attendance sheet: The attendance sheet is made by the department according to the actual attendance days, and has the signature and signature stamp of the attendance sheet。
3. Comprehensive evaluation form of training physicians (provided by physicians only) : scored by the training department, signed by the director of the department and the teacher, one for each department;
4. Information form of practicing physician (only provided by physicians) : need to be stamped by the unit;
5. Evaluation form for nursing practitioners (nursing provision only);
6. Practitioner's (nurse's) license and qualification certificate;

2. Provide a one-inch standard ID photo。

Note:

① The date of issuance of the completion certificate is: the date of the completion certificate;

② Students who have studied for less than 3 months, who have been suspended in violation of the requirements of our hospital, and who have failed the department evaluation and appraisal will not be issued a certificate of completion;

③ After the certificate of study completion is sealed by our office, it is necessary to go to the hospital office (Room 308, 3rd floor, Building 6) to seal the hospital seal。
